CASE MANAGEMENT

​

Twin Oaks Community Services provides comprehensive Dependency Case Management services for children and families involved in Florida's child welfare system. Working in partnership with the Florida Department of Children and Families and Northwest Florida Health Network, our case managers coordinate services that protect children, strengthen families, and help children achieve safe, permanent homes.

​

Whether children remain safely in their own homes or are placed in out-of-home care, our team works with families from the beginning of their involvement until permanency is achieved.

​What we do

​

Our case managers serve as the primary point of contact throughout a family's involvement in the child welfare system. From the earliest stages of engagement through permanency, we coordinate services, advocate for children, facilitate communication among partners, and help families navigate complex systems of care. Working collaboratively with parents, caregivers, schools, healthcare providers, behavioral health professionals, the courts, and community organizations, we ensure children receive the comprehensive support they need to thrive.

 

Our services include:

  • Individualized case planning

  • Regular home visits and family engagement

  • Court reports, court participation, and judicial support

  • Permanency planning and family reunification services

  • Coordination of medical, dental, behavioral health, educational, and developmental services

  • Collaboration with schools, healthcare providers, and community partners

  • Advocacy for children's safety, well-being, and permanency

  • Ongoing case monitoring and progress reviews

  • Connections to community resources and supportive services

By combining compassionate support with professional expertise, we help families overcome challenges, strengthen protective factors, and create positive outcomes that last well beyond the conclusion of case management services.

Our family-centered approach recognizes that parents and caregivers are essential partners in achieving positive outcomes for children, and we are committed to engaging them with dignity, respect, and compassion throughout the child welfare process.

 

We work alongside families—not simply on their behalf—to build protective factors, strengthen relationships, and develop the skills and supports needed to create safe, stable, and nurturing homes. By focusing on each family's unique strengths, culture, and circumstances, we develop individualized service plans that encourage meaningful participation, shared decision-making, and long-term success.

 

Our case managers collaborate with parents, relatives, foster caregivers, schools, healthcare providers, behavioral health professionals, and community organizations to coordinate services that address the whole family's needs. Whether supporting a family in safely maintaining children in the home, working toward reunification, or pursuing another permanency goal, our focus remains on creating lasting, positive outcomes that promote child safety, family well-being, and lifelong stability.

permanency planning

 

Every child deserves the security, stability, and sense of belonging that comes from having a permanent family. Permanency planning is at the heart of everything we do, guiding our work from the first day a child enters the child welfare system until a safe, lasting home is achieved.

 

Our case managers work closely with children, parents, relatives, caregivers, the courts, and community partners to develop individualized permanency plans that reflect each child's unique circumstances, strengths, and needs. We continually assess progress, coordinate services, and remove barriers that may prevent families from achieving timely permanency.

 

Whenever it can be accomplished safely, reunification with parents remains the preferred permanency goal. When reunification is not possible, we actively pursue other permanent family connections through adoption, permanent guardianship, placement with relatives, or other legally established permanency options. Throughout the process, we are committed to preserving important family relationships, maintaining sibling connections whenever appropriate, and ensuring that children experience as much stability and continuity as possible.

 

Our focus is not simply on finding a placement, but on helping every child achieve a safe, nurturing, and permanent home where they can grow, thrive, and reach their full potential. 

SUPPORTING CHILDREN IN FOSTER CARE

 

Children in out-of-home care require more than a safe placement—they need opportunities to heal, grow, and thrive. At Twin Oaks Community Services, we work closely with caregivers, healthcare providers, educators, and community partners to ensure every child receives the services and support necessary to promote their physical, emotional, educational, and developmental well-being. Our case managers advocate for each child's individual needs while helping maintain important family connections and minimizing disruptions in their lives whenever safely possible. We believe that stability, consistency, and meaningful relationships are essential to helping children build resilience and achieve positive long-term outcomes.
 

Children in foster care receive coordinated support that includes:​​

  • Behavioral health services

  • Developmental services

  • Opportunities to participate in extracurricular and community activities

  • Stable placements that minimize disruptions

 

We also work to preserve important family and sibling connections whenever it is safe and appropriate, recognizing that these relationships play a vital role in a child's sense of identity, belonging, and long-term well-being.
