High Fidelity Wraparound is an evidence-informed, family-driven planning process that brings together the people and resources needed to help children and families achieve their goals. At Twin Oaks Community Services, we partner with families to build individualized plans of care that reflect their strengths, priorities, and vision for the future.
Rather than focusing on a family's challenges alone, High Fidelity Wraparound identifies strengths, natural supports, and community resources that can be leveraged to create lasting solutions. Families lead the planning process while our Wraparound Facilitators coordinate services, organize team meetings, and help ensure every member of the team is working toward shared goals.

High Fidelity Wraparound is a collaborative process that helps children with complex behavioral, emotional, or mental health needs remain safely at home and in their communities whenever possible. Through a structured, team-based approach, we work with families to develop comprehensive plans that coordinate services across multiple systems while promoting long-term stability and success.
Our Wraparound Facilitators serve as guides throughout the process, helping families navigate services, identify resources, and bring together the people who can best support the child and family. Every plan is individualized, strength-based, and designed to address the unique needs and goals of each family.

Families are the experts on their own lives. High Fidelity Wraparound places families at the center of every decision, ensuring their voice, values, and priorities guide the planning process.
Rather than developing plans for families, we develop plans with families. Parents, caregivers, youth, extended family members, friends, educators, service providers, and other trusted individuals work together as one team to identify strengths, overcome barriers, and celebrate progress.
This collaborative approach encourages shared ownership, strengthens relationships, and builds confidence as families work toward achieving their goals.
STRENGTH-BASED APPROACH
Every family has strengths that can serve as the foundation for positive change. Our Wraparound process begins by identifying those strengths and using them to develop creative, individualized solutions that build resilience and promote long-term success.

YOUTH & FAMILY VOICE
High Fidelity Wraparound is built on the belief that meaningful outcomes occur when children and families have an active role in making decisions that affect their lives.
Youth are encouraged to share their goals, interests, and perspectives, while caregivers help shape the vision for their family's future. Every team member's voice is valued, but the family's voice guides the process.
This commitment to family voice and choice promotes greater engagement, stronger relationships, and plans that families are more likely to embrace and sustain.
CARE COORDINATION
Families often receive services from multiple agencies, providers, and systems. Without coordination, navigating these services can become overwhelming.
Our Wraparound Facilitators help ensure everyone involved in a child's care is working together toward common goals. We coordinate communication, facilitate regular team meetings, monitor progress, and help resolve barriers that may prevent families from receiving the services they need.
This coordinated approach reduces duplication of services, improves communication, and creates a more seamless experience for children and families.
NATURAL SUPPORTS
One of the defining features of High Fidelity Wraparound is the emphasis on building lasting support systems within the family's own community.
In addition to professional services, we help families identify trusted relatives, friends, neighbors, faith communities, mentors, coaches, and other individuals who can provide ongoing encouragement and assistance. These natural supports often become the foundation for long-term success because they remain involved after formal services have ended.
By strengthening these relationships, families develop greater confidence, resilience, and independence.
TRAUMA INFORMED
Many children and families participating in High Fidelity Wraparound have experienced significant adversity, trauma, or other life challenges. Our facilitators recognize the impact these experiences can have on emotional well-being, relationships, and daily functioning.
Every interaction is guided by a trauma-informed philosophy that promotes emotional safety, trust, collaboration, and respect. We strive to create environments where children, youth, and families feel heard, valued, and empowered to make decisions about their own lives.
